    * Make Sure your FORM/Chunk names follow the naming conventions in
      the document EA IFF 85 Standard for Interchange Format Files
      (available from Amiga) and also in the 1.3 RKM: Includes &
      Autodocs IFF Appendix.

    * Email Amigan Software at jrj76@optusnet.com.au to find out
      if your FORM/chunk name conflicts with any existing name.

    * Fill out this form.

    * Send this form by email, with your ASCII text specification,
      include files and sample source code as attachments, to
      jrj76@optusnet.com.au.
